These steps you have to follow to apply for the new voter identity card online, You can simply apply for fresh voted ID you have no Id previously, also you can apply if you are changing your constituency.

Step 1:

Click here to visit the website . http://www.nvsp.in/ . You will see a website as shown in below image

STEP 2 : 

If you want to apply a fresh Colour VOTE ID then click on the SECOND LINK "Apply online for registration of new voter/due to shifting from AC" . You will see a FORM-06 as shown below.

Form 06 of Voter ID apply has SIX parts, You have to fill all the details , then you can submit the form

 Part -01 

Here you have to enter the following details

1. Sate
2. Parliamentary Constituency
3. Choose whether you  are applying for first time or due to shifting from another constituency

 Part -02 

This part is a  Mandatory Particulars, here you have to give your details like

1. Name of the voter (Both in English/Regional Language)
2. Surname of voter (Both in English/Regional Language)
3. Name of the Relative Applicant 
4. Surname of  the Relative Applicant 
5. Type of Relation (Father/Mother/Husband/Wife)
6. Date of Birth/ Age as on date mentioned in the form
7. Gender of applicant

 Part -03 

In this part you have to enter the details of Current address where applicant is ordinarily resident. The details are

1. House No.: 
2. Street/Area/Locality  
3. Town/Village 
4. Post Office  
5. Pin Code 
6.  State/UT / District 
7. Similarly you have to enter the permanent address
8. Enter (j) EPIC No. (if issued) 

 Part -04 

This part of the FORM-06 is an Optional Particulars, You may or may not enter these details

1. Enter Disability status ( if any)
2. Enter your Email ID (Optional)
3. Enter Mobile number

 Part -05 

In this part of the form you have to upload the Upload Supporting Document in JPEG/JPG format. The size should be below 100kb

1. Photograph
2. Age Proof (HSC Certificate/Aadhaar Card/PAN Card)
3. Address Proof (Aadhaar Card/DL/)

 Part -06 

The last part of the form is declaration. Here you have to enter the details like

1. Village/Town of resident
2. State
3. District
4. Date of apply

Then Click on the  SUBMIT  button. You will get a link  to check the status of your VOTER ID card online.
